Is Leg Pain at Night a Vein Problem?

Leg pain related to venous disease is often underestimated. Many patients attribute their symptoms to muscle fatigue, arthritis, or restless legs — without knowing that chronic venous insufficiency is the underlying cause.

Venous leg pain has a characteristic pattern:

• Aching, heaviness, or throbbing — worsens throughout the day, especially after prolonged standing or sitting
• Nocturnal cramps — sudden, painful muscle spasms in the calf at night, often awakening the patient
• Restless legs — an irresistible urge to move the legs at rest, particularly in the evening
• Relief with elevation — symptoms improve when lying down with legs raised above heart level

Why Do Veins Cause Leg Pain?

When vein valves fail, blood pools in the lower legs (venous stasis). This causes venous hypertension — elevated pressure within the vein system. The excess pressure stretches vein walls, activates pain receptors, and creates the inflammatory environment that produces aching, cramping, and throbbing.

As venous insufficiency progresses (CEAP C3→C4→C5→C6), skin changes, venous eczema, and ultimately venous ulcers can develop — making early treatment important.

Distinguishing Venous Pain from Other Causes

Not all leg pain is venous. Arterial insufficiency (PAD), sciatica, knee osteoarthritis, and true restless legs syndrome all produce leg discomfort. The key distinguishing features of venous pain are its day–night pattern, improvement with leg elevation, and association with visible varicosities or skin changes.

Diagnosis: Venous Duplex Ultrasound
