Chronic venous insufficiency (CVI) is a condition in which the leg veins fail to return blood efficiently to the heart. It affects 25–40% of adults at some stage of life. Without treatment, venous disease worsens progressively — but modern minimally invasive procedures close the diseased veins permanently, with same-day return to activity.
What Is Venous Insufficiency?
In a healthy leg, blood flows one-way from the foot toward the heart — against gravity — thanks to one-way valves inside the veins. When these valves weaken or fail, blood falls backward (called venous reflux) and accumulates in the lower leg veins. The elevated venous pressure damages vein walls, the surrounding tissues, and eventually the skin — producing the symptoms of CVI.
CEAP Classification — Stages of Vein Disease
Venous disease is classified internationally using the CEAP system. Understanding which stage you are at helps clarify the urgency and type of treatment needed:
- C0 — No visible or palpable signs of vein disease
- C1 — Spider veins (telangiectasias) or reticular veins — fine red or blue networks
- C2 — Visible varicose veins — bulging, tortuous veins above 3 mm
- C3 — Leg swelling (edema) without skin changes — ankle puffiness by evening
- C4 — Skin changes: brown discoloration, venous eczema, skin hardening (lipodermatosclerosis)
- C5 — Healed venous ulcer
- C6 — Active open venous ulcer — the most advanced stage, requiring urgent treatment
Most patients who seek treatment are at C2–C3. Skin changes (C4) and ulcers (C5–C6) indicate advanced disease and carry higher treatment complexity.
Symptoms of Venous Insufficiency
- Leg heaviness, aching, and fatigue — worsening after prolonged standing
- Ankle and calf swelling, most pronounced by evening, improving overnight
- Itching or burning sensation over varicose veins
- Nocturnal leg cramps and restless leg symptoms
- Visible varicose veins or spider veins
- Skin changes around the ankle: darkening, thickening, eczema
- Venous ulcers — open wounds near the inner ankle that fail to heal
Diagnosis: Venous Duplex Ultrasound
The gold standard for diagnosing venous insufficiency is the venous duplex ultrasound — a non-invasive scan that maps the venous system and identifies exactly where reflux is occurring and in which veins. Is Venous Insufficiency Dangerous?
Left untreated, CVI progresses. Skin changes (C4) impair quality of life significantly; venous ulcers (C6) can become chronic wounds requiring prolonged wound care. There is also elevated risk of superficial and deep vein thrombosis (DVT). Early treatment prevents progression to these advanced stages.

Is chronic venous insufficiency dangerous?
Without treatment, CVI can progress to venous ulcers — open wounds near the ankle that are difficult to heal. There is also elevated risk of superficial phlebitis and deep vein thrombosis (DVT). Early treatment prevents progression to these advanced stages.
Can venous insufficiency be cured permanently?
The diseased veins can be permanently closed. EVLA and VenaSeal close the refluxing veins definitively. Because there is a genetic component to the disease, lifestyle measures — maintaining a healthy weight, regular exercise, and wearing compression stockings during prolonged standing — help reduce the risk of developing new venous disease.
What is the difference between varicose veins and venous insufficiency?
Varicose veins are one manifestation of venous insufficiency (CEAP stage C2). Venous insufficiency is the underlying disease — dysfunction of the vein valves — of which varicose veins, spider veins, leg swelling, and venous ulcers are all different stages.
How is venous insufficiency diagnosed?
The only reliable way to diagnose venous insufficiency is with a venous duplex ultrasound. Symptoms such as leg heaviness and visible veins are strong indicators, but the ultrasound identifies exactly which veins are responsible. Is RFA treatment covered by kupat holim?

How long is the recovery after RFA?

EVLA — Endovenous Laser Ablation
The international gold standard for treating venous insufficiency. A thin catheter is inserted into the diseased vein under ultrasound guidance; laser energy delivered along the vein length causes it to close permanently. Performed at Shamir Medical Center via kupat holim tofes 17 — eligible patients receive treatment at little or no direct cost. Closure rate at 1 year: 95–98%.
VenaSeal — Medical Adhesive
A medical-grade cyanoacrylate adhesive is injected to seal the diseased vein — without tumescent anesthesia and without post-procedure compression stockings. VenaSeal costs ₪15,000 and may be covered by supplemental private health insurance. Fastest return to sport and exercise of any procedure.
RFA — Radiofrequency Ablation
Similar technique to EVLA but uses radiofrequency heat instead of laser. Performed at a private hospital through supplemental private insurance. Closure rates comparable to EVLA.
Sclerotherapy
A sclerosing agent injected into smaller varicose veins and spider veins causes them to close gradually. Used alone for spider veins (from ₪1,500 per session) or combined with EVLA/VenaSeal for residual varicosities (from ₪5,300 for venous insufficiency indication). See our sclerotherapy guide.
