EVLA vs RFA — Laser vs Radiofrequency for Varicose Veins
EVLA (endovenous laser ablation) and RFA (radiofrequency ablation) are the two most widely used thermal ablation techniques for treating varicose veins and venous insufficiency. Both work on the same principle — controlled heat delivered from inside the vein — but they differ in the energy source, catheter design, and procedural experience.
How Each Works
EVLA uses a laser fibre (typically 1470nm wavelength) inserted through a skin puncture into the diseased vein. The laser emits controlled pulses of light energy that heat the vein wall to the point of thermal coagulation. The vein contracts and is then absorbed over 3–6 months.
RFA uses a radiofrequency catheter (such as the ClosureFast system) that delivers alternating electrical current to heat the vein wall in 20-second treatment segments. It works in the same fundamental way as EVLA but via electrical rather than optical energy.

Which Is Better?
Systematic reviews comparing EVLA and RFA (including the EVRA trial) show equivalent efficacy at 1 and 5 years. The choice between them in clinical practice is primarily driven by:
- Insurance coverage: In Israel, EVLA at Shamir Medical Center is the tofes 17-eligible procedure. RFA is available privately only.
- Vein anatomy: Very tortuous veins may be technically easier to treat with laser fibre flexibility than the more rigid RFA catheter.
- Physician experience: Each technique requires specific training; outcomes are most influenced by operator volume and skill.
