Why Do My Ankles Swell in the Evening?

Leg and ankle swelling (edema) that accumulates during the day and disappears after a night’s sleep is one of the most recognizable patterns of venous-origin edema. When vein valves fail, blood and fluid leak into the surrounding tissue, causing the characteristic swelling that worsens with prolonged standing or sitting and improves with leg elevation.

Unlike cardiac or renal edema — which tends to be bilateral and persistent — venous edema often starts in one leg, worsens in warm weather, and is more pronounced in the ankles and lower calf.

Signs That Point to a Vein Cause

• Swelling is worse at the end of the day and after long periods upright
• Swelling resolves or markedly improves overnight
• Swelling is accompanied by leg heaviness, skin itching, or visible varicose veins
• The skin around the ankle appears brownish or discolored (lipodermatosclerosis)
• Shoes feel tight by evening

When to See a Doctor Urgently

Seek prompt medical attention if leg swelling is accompanied by calf pain, warmth, and redness — these may indicate deep vein thrombosis (DVT), a condition requiring immediate treatment.

Diagnosis: Duplex Ultrasound
